Normal Work Schedule: Monday - Friday 8:00am to 4:45pm; some evening/weekend hours as needed.


Job Summary: The Clinical Documentation Associate supports clinical workflows by serving as a bridge between patients and healthcare teams. Works with patients and providers to ensure a complete and accurate medical record. Manages data relating to patient care outcomes for reporting. Identifies potential gaps in clinical documentation for inpatient and payer populations. Supports process improvement initiatives.


Additional Department Summary: Performs liaison duties for Family Internal and Rural Medicine (FIRM) clinic patients. Supports overall quality improvement initiatives and completeness of clinical documentation to accurately reflect patients' care status through extensive interaction with physicians, nursing staff, other patient caregivers, and coding staff. Assists with case management and identifies opportunities for improvements/enhancements to documentation tools, provider engagement, and/or processes related to documentation and related outcomes. Contributes to the education of medical residents, medical students, nursing students, new employees, and other LPN's. Facilitates quality population-based patient care within the scope of licensure.


Required Minimum Qualifications: Licensed Practical Nurse (LPN) in the State of Alabama in good standing and two (2) years of LPN experience.


Additional Required Department Minimum Qualifications: Current CPR certification. Experience using an electronic health record system in a previous learning or employed role.

Final candidates must submit to and successfully pass a post-offer drug screen.


Skills and Knowledge: Ability to perform patient care as directed by a physician, nurse practitioner, or other qualified medical professional. Efficient and accurate computer and data entry skills. Excellent communication, public relations, and customer service and telephone etiquette skills. Ability to multi-task in a fast-paced work environment; Ability to exercise discretion, diplomacy, and tact in patient/staff interactions. Willingness to learn and consistently deliver patient-centered high-quality customer-friendly service. Ability to maintain strict confidentiality of protected health information and follow HIPAA regulations at all times.


Preferred Qualifications: Five (5) years clinical nursing experience, to include some supervisory experience in a medical office/clinical setting. Familiarity with population health software and value-based care preferred.
